Well...
I don't buy the idea that Akio killed Utena's parents. It's just not like him.
He DID brainfrell Nemuro into killing 100 duelists during the BRS, but that's different: they had a contract.

Oh, even suppose jiko = accident and cannot mean incident, it needs not be narrowed down to a car accident.
A planned household natural gas leak could easily off a couple; a home propane explosion could erase a lot of the murderer's traces.
Or, an arson ala Nemuro Hall . . . it seemed like the official records never could pin Nemuro down as the culprit.
All the above is supposing that the Tenjou couple were actually prior involved with Ohtori (as the rose coffin might hint at). Suppose Akio simply approached Utena without prior planning (except for the "funeral details"), then their meeting would be totally random, meaning that Akio actively roams all around the outside world targeting various children he see as having pawn/duelist potential.

We could speculate all day I guess the bottom line is that -- unless we're missing a line somewhere or there's more to the translation than I can understand -- we don't have enough information to pin down a cause of death for the Tenjous. Even Ikuni may not know how it happened, and if he had wanted us to know, it would have been easy for him to tell us in the text (by having the guy with the flashlight say "car accident" or "explosion" or "Angel attack"). So it's safe to conclude that their cause of death is immaterial as far as subtext is concerned. We can fill that gap with whatever fic bunnies we want.

It would be beautifully symbolic if they drowned. After all Utena tries to drown herself, Juri tries to drown her locket, and lots of important discourses are made while people look out over the water.
