What is difference between first fight and thid fight?
Utena and dualist fight first time each. we can meaning of that at Episode 13.Akio show and review the fight's meaning
and over the mikake's fight next they fight each again. what's mean of this fight?

Any number of other people could answer that question better. But all of Utena's duels against Touga seem to be duels of identity just as the ones against Saionji are tests of raw courage.

Adding to what brian said, all of Utena's duels with Miki seem to be about morals, specifically about Anthy's role as a possession. Juri's duels... secrets? True love? Breaking free?
