There's actually a piece in the gallery that shows how it was done. Personally though, since we do see the back of Utena's head with her hat off (to the best of my recollection) and that pile of hair isn't there.... I'm inclined to think it's magic. Like how Touga's alive in the movie.

............................... Wow. I've never noticed how movie!Utena's breasts are far more pronounced in her shiny duelist outfit than in her normal school outfit. BUT. That's beside the point.
